I read this book a few weeks ago, and the incredibly strange narrative and construction inspired me to create an interesting book cover design. The book is centered around the life of Nazi doctor during and after the Holocaust. Amis tells his story backward: starting with his death and ending with his birth. Everything Tod Friendly does he does in reverse, which inspired the reflected dust cover design. The photo used is an actual photo of Josef Mengele, a doctor who experimented on Jews in the Holocaust. 
Many modern historians surmise that Nazi doctors of the Holocaust had split personalities to be able to inflict such pain and destruction and still be able to live "normal" lives. The scribbles inside Mengele's split head visually represent this dichotomy.
